This same question comes up about once every six months. Last time, I got curious and started researching. The biggest problem was that there is not one place to find the information. Sometimes I think the HR Departments at some of these companies do not even know exactly how many employees they have. People come and go every day. They get laid off and hired in groups, depending on whether the MTSO gains or loses clients, and then there are the buy-outs and mergers.
I finally came up with some numbers, but have no clue how accurate or current the information is. It looks some the data-collecting companies used several methods to obtain the information, since some numbers are in large ranges (like someone was asked to check off boxes on a generic questionnaire) and other numbers are exact. Some information was probably taken from the Company’s SEC Filings, while other information was probably obtained from a press releases and interviews (no way to know who filled out the questionnaires and whether they had access to accurate information) and nowhere does it give any dates.
Because of the large ranges ($10 to 50 million in revenue, 1000-5000 employees, etc), some of the MTSOs had to be lumped together, as I am sure some are at the high end, while others are at the low end. There is simply not enough information to try to refine the order any more accuraely. Not to mention the large conglomerates with tentacles in other areas, like Focus (A Subsidiary of Nuance) and Fast Chart (Div. of Applied Medical Services). There is no way to know if the numbers are for the Transcription division only, or if they include other parts of the parent company as well.
The top 4, however, are pretty much accurate as far as rank because they brag about their standing every chance they get. “We are the largest – we are the second largest – we are the third largest – we are the fourth largest - medical transcription service provider.” Easy to Google them. LOL Exact numbers, on the other hand, are anyone’s guess.
With parts of MedQuist and Spheris now merged with CBay, there is no way to know how that affected employees, or which name will ultimately be used when they all get lumped in together.
So take the following information as an estimation of probably inaccurate information to begin with. If anyone has contradictory (accurate) information, please do post your figures so we all have a better idea of where these MTSOs stand.
I tried to find information on the top 15 MTSO that KLAS published, and then added a few of the MTSOs who recently had job opening ads.
I added another category just for laughs. The site I was looking at had a page for “suits.” -- lists of the names of the Owner, CEO, CFO, VPs, Managers, Directors, IT hot-shots, etc. When the “suits” outnumbered the listed number of total employees, I knew they were way off. Although, I am sure the “suits” take home 90% of the revenue, so the revenue number was probably closer to actual.
In case these lines do not hold their positions after I send the post, here is the order:

The following employee numbers just simply did not make sense, considering the number of “suits” listed, so I am not going to try to arrange them in order. They all seem to be about the same size.

To no surprise, 13 of the 15 KLAS “Top 15” off-shore to India and the Philippines, including 3 of the top 4 largest MTSOs. It will be interesting to see in six months or a year down the road if the big MTSOs get bigger and gobble up the smaller ones and/or if the smaller MTSO who do NOT off-shore can stay in business without going overseas.
