What you are missing is that you don’t know whether those short notes belong to Medicare patients and if so whether Medicare paid close claims. I used to be an MT and now work in clinical documentation. The rules have not changed and claims still get denied for missing information and coding errors. This is true not only for Medicare but all insurance providers. If providers refuse to create their own charts the facility won’t get paid either.
