They used to request records and if the dictation didn't include the data to support the claim they would reduce payment or not pay. I guess they don't do that anymore. Some of the records I see now are so short and missing information that I consider them a malpractice attorney's dream - and a patient's potential nightmare. All to save a dollar on transcription. Doctors, nurses, etc complain about having to create their own charts but won't refuse to do it so CFO/administrators don't care. SMH
