Are okay with grossly erroneous dictation but heaven forbid if a transcriptionist forgets a coma or a "the or an a" and our pay is docked. Yes, we make errors, even missing a word every now and then but not throughout the entire report. I do a lot of oncology reports and when it comes to putting in staging and grades, etc., the editor will put in something completely different. Scary that that it is in the permanent record and according to the powers that be, they cannot make corrections. What is wrong with this picture?
