I went to only using the speaker quite some time ago too. The noises, hissing, etc, are too physically painful. I again had several dictations in a row by one of the physicians whose dictation always has a hissing noise throughout. Afterwards, it affected my ability to hear for the next several jobs. I have turned down the volume as much as possible, and just blank what could probably be heard if I turned up the volume. I am too tired to put up with the abuse.
