Scientific and medical journals have - different criteria

for articles submitted for publication. One is the AMA Manual of Style. They may have different rules for how numerals are expressed. I'm including a Wikipedia link regarding a number of style guides. Look on the far right of the article. There are many, many style guides, so the journal you are writing for will specify which guidelines you must follow. It's got nothing to do with being modern or old fashioned.

As far as medical clinical documentation, whether it be transcribed into a medical record or handwritten, it now requires numerals under 10 to be expressed in the Arabic form with a few exceptions.

That's all there is to it. End of story. Of course, if you have an old fashioned account who wants the numbers spelled out, no one is going to come and arrest you or fine you, so have at it. Just know what the modern standards are if you need to use them.
