I am on a extremely busy huge hospital system up in Toronto, Canada. No set schedule. Since I had a full-time job when I applied, they only asked for a "general" time I would be working, i.e., morning shift, afternoon, evenings, nights. Line requirement minimum is 500 lines per day. Tough, tough account to get used to; however, very interesting to me and very educational. Incentives when the account is extremely busy, 0.055 per line if one works 6 hours. Slow start lph with such a learning curve so, of course, very low salary - getting better. 0.04 per line Editing, 0.07 per line straight transcription. They have an Elite team where if you commit to 1500 lines per day 5 days per week, line rate is 0.045 per line. Here is where to send your resume if interested.
