Depends on what's being measured. One site mentions breast cancers on imaging to be expressed in length, width, and height measurement. So it might also depend on the specialty. Whatever is dictated should be the way it is transcribed, as we assume that the dictator knows and those reading their reports know the order and value of the measurements they are talking about.
