First, with the information you have given for retroperitoneal carcinoma, I would code it as C48.0 because I have nothing else to go by.

Second, pertaining to the question asking for five secondary malignant sites when there is a malignant neoplasm not otherwise specified, the five secondary sites would be considered mets in this question. So, you go to the list in the AHA Handbook, and pick five of them and code them as secondary or mets along with C80.1 (unknown primary malignancy).

Example of five sites would be: bone, brain, pleura, mediastinum, and heart.
1. Bone would be secondary mets = C79.51 + C80.1 (unknown primary)
2. Brain would be secondary mets = C79.31 + C80.1 (unknown primary)
3. Pleura would be secondary mets = C78.2 + C80.1 (unknown primary)
4. Mediastinum would be secondary mets = C78.1 + C80.1 (unknown primary)
5. Heart would be secondary mets = C79.89 + C80.1 (unknown primary)
