To anyone who is considering applying for this, please read archive information. She wanted me to sign a contract and send all personal info before testing. I told her I wasn't going to do that, and only gave my name and address. I passed the testing and was contacted by some unknown person who said she subcontracted from him. She gave out my PHI to this person without my permission and I had to contact my attorney about it. Be very careful.
