If I have the slightest doubt I QA it and will not take a hit for that. The numbers are so hard to decipher at times. The blood pressure in voice was 112/70 and I left it like that as I thought that is what I heard too, but it was 102/70. I got dinged big time for that. Now when I come across vital signs, dosages and labs, I listen very carefully and QA if in doubt. We are always held responsible for everyone else's mistakes, including the voice recognition, which is a total disaster. I bet if I had been transcribing that report, I would have caught that BP.
