Some are single days off and others are a week at a time or 10 days. I do let my company know as far in advance as a courtesy.
I work for a company who knows that a true IC doesn't have a set schedule, although since I usually work afternoons M-F, if I don't plan to that day, I let them know. Also, if I want to jump on for some extra work on the weekend, I usually let them know too.
I've never had an issue with time off or asking "permission". I just let them know what dates I'll be off and they thank me for the notice.
Every now and then we'll get emails that a few ICs are on vacation at the same time and they will ask for extra help on that account to keep it in TAT, which is never a problem.
