I have had years where I was missing a 1099 as an IC and my accountant filed my taxes based on my totaled checks for the year. If you have a final pay stub that shows total pay and total deductions, that should work. You can make a notation that you were not provided a W2 from your employer.
It's IMEDX's responsibility to have provided the W2, not yours.
You can only provide the most accurate information that you have.
