I know a transcriptionist is going to hear it anyway at some point, but I'd really prefer there not be anyone else in the room listening when I'm upset and in tears and feel completely humiliated over something.
You want your personal issues open for everyone to listen to, then invite the whole clinic into the exam room to listen in.
