What you say is probably partially true, but I don't think 75% of the doctors are originally from foreign countries at this moment, so if 75% of the dictators sound like they're from foreign countries, mostly sounds like from India, that must mean that Nuance is discriminating against U.S. MTs, purposely sending U.S. MTs the more hard-to-decipher speakers because people still need jobs and can't leave right away and thus they can treat us however they want, even try to take advantage of us by giving us the more difficult work whereas Indian MTs get all the easier clear-speaker and/or non-ESL speakers because the Indian MTs themselves are English as a second language and thus will not be as proficient at deciphering English even if it's spoken in an Indian accent.
