Hx = "large anterior-posterior labral tear, as well as anterior superior chondromalacia, greater trochanteric bursitis of the right hip". 

Context = Under "extremities" on PEX:  "...a less positive FADIR (flexion, adduction, internal rotation, extension) exam.  Negative "wiper" exam."

I usually hear it referred to as windshield wiper. Google "shoulder exam, windshield wiper." It gives a good explanation with the first result.

Sorry, don't ask me why I thought you were referring to the shoulder, but this also refers to the hip, can be found on google and in Stedman's ortho book as windshield wiper effect or windshield wiper sign.

Thanks for that.

I was swayed by the FADIR acronym used earlier in the report, so was expecting "wiper" to be another acronym. Will look for windshield wiper effect/sign.

The first time he said it, I, too, thought it might be "weight-bearing" but he spelled it. I think lexluther (above post) is right.

Regarding the positional terms, thanks for the heads up. I'm aware that those can be used in their combined forms (i.e., anterosuperior, etc.) but in this case, the physician is clearly stating them as separate words (i.e., anterior superior).

this note is for pain of the right ankle - She has had multiple treatment modalities, including s/l "an o fa reeses", Lidoderm patch, acupuncture, Neurontin. and on physical examination, There are no skin changes over the ankle.  No erythema, warmth, or other sign of infection.  There is no s/l "al o din e um".  She does not have a focal area of tenderness to palpation. ...
