Part of what you're hearing might be "white matter disease", but without context I can't figure out the "birdno".

It would really, really help to know what words come before the mystery words. It would also help to know if we are talking cardiology or neurology.

It is neurology. MRI of the brain with gadolinium. Here is the paragraph.
Postsurgical changes of the left frontal lobe with subacute hemorrhage and dural enhancement at the operative site. Enhancement has decreased since a previous exam. This is likely due to postsurgical changes. Small meningioma along the anterior falx. Old right parietooccipital infarct. Moderate ___ consistent with chronic ischemia.
It is a new ESL. I have never transcribed for him before and it's a bit challenging. I do appreciate any help.

It was actually the word "moderate" at the beginning of the sentence that made it click for me. Glad it worked out!
