Cant we all write the guys who are on the e-mail we all got if in fact this turns out to be something bad? Flood their emails and desks? Ya know its too bad we cant get together 500+ MLS and go on strike... I would do it. If a ton of us went on strike and demanded fair wages and changes, with noone typing their reports for 48+ hours I think they'd listen. It kind of sounds like the pay will be the same even if they lower the base..... b/c if they lower the base rate but you get a tad extra for weekend/night work and also good quality documents, it seems it would even out, no? Also- I work weekends and I do not get any kind of shift differential. Never have. Only nights I see it on my pay and that is when i'm helping out.

Funny thing I e-mailed my TM asking questions and as usual when it comes to something like this, no response:( One thing I have found is there is absolutely NO communication with this company.

Typical lack of communication. If Nuance had done this In-Touch thing as they claim, I find it hard to believe that lack of communication was not right up there at the top of the list. The lack of communication is consistent when it comes to dealing with management.

They obviously know something they're not - telling, & have been told to keep mum.

secret that more and more of the work is being sent offshore these days and according to my TL is set to increase even more in 2015. I hope it all backfires on these companies. Eventually, when India and elsewhere is doing the majority of transcription, they will start demanding higher wages and then what?

You know, I have thought exactly along your lines. What this field needs and has needed for a long time is a "union", but it has to have enough people "in" to strike in order to be effective. Perhaps a poll to reach across the boards here to see if enough slave laborers would sign on to make an impact.

I think UPenn had a union and I KNOW UNM did and that didn't stop those facilities from outsourcing to Nuance.

What makes you think a union would stop Nuance from offshoring completely?

US MTs, offshore MTs, and even VR are going - to all be replaced by point-n-click.

It's already happened big-time in radiology transcription, and is quickly spreading to the other specialties as well. I'm a big union believer, but the problem with unions is, they only work if they have a profession to support. Our profession is 99% gone.

has been closing MT departments right and left. My hospital went to it last year and laid off their entire MT department. It's an electronic medical record (EMR) software program whereby the doctor doesn't even have to dictate anymore. He or she simply pulls up a standardized template on the screen, and then points at and clicks on whatever he or she wants to add to the template. Most docs these days are completely computer-savvy, and no longer have such an aversion to entering their own data. So when my hospital went to this type of software, they bypassed the usual step of outsourcing their work to an MTSO. They simply had a sudden series of calls to all their MTs (in various different hospitals belonging to this HMO) and terminated our employment. Took all of 10 minutes.

My health system (I work inhouse from home) recently completed a migration to Epic across 15 clinics and 8 hospitals statewide. Because I have really, really good managers, when the Epic training people said "you don't have to dictate at all," my managers stepped in and said "no, you don't, but if you WANT to, we'll show you how."

It decreased our total transcription volume by 70%, but because so much of what we do was outsourced to Nuance, we were able to bring it all back in house and we are actually adding 4 positions to the department. Even a lot of the young docs are dictating the partials for the assessment and the HPI and we do those as partials.

I know my experience is out of the ordinary, but it is possible and I believe more places will go back to it when they realize how bad the alternative is proving to be.

I don't know why I think a union would help, just a desperate thought, I guess. One will try anything when in the throes of drowning. To be honest, I am actually one of those who thinks nothing is going to help this field now. I was responding mainly to the idea of a strike a couple blogs up and did not see anyway to organize that other than a union. Reflecting my age, huh?
Averting strikes is probably one of the major - reasons MTSOs keep MTs incommunicado.

Forgive me too for thinking that was not to me, since in the past they have treated us hardly as colleagues, but I guess they did mean to send it to us.

Is that because it was to TSMs and QA/QC too, or is it just because they want us to feel one big happy family - to my knowledge nobody's actual pay gets affected by this except MTs, but if anyone knows different please tell.

It has some good talking points, but because it sounds so complicated it is worrying, in that I am worrying they are hiding something in those fancy graphs that is bound to make it harder to make a living wage, no matter how good your quality or quantity.

You know these corporations never give back once they take away, right? Except for the top management layer, they get plenty of raises, bonuses, etc., with apparently no accountability at all.

I cannot imagine they would now increase the amount they pay to MTs in total, so one wonders how/if the distribution is going to change and just how bad that will be.

Or who knows maybe they will turn around and become a decent employer not seemingly trying to screw us out of our last dime. One can only hope, right?

past. I applied here for an MLS position 9 months ago and the recruiter suggested I would be a better fit for a QC position instead. They talked about a QC bonus plan at the time but that never came about. I took the QC position with the agreement if I didn't like it I could move to an MLS position. I think that is going to be my plan at this time.
If QC pay is production-based, that will make - the MTs lives a living nightmare.

the QC Comma-Police are doing already. Taking the off hourly and putting them on production means they'll have to find even more "mistakes" (real or imagined) in order to make their own quotas. This at a time when MT pay is poised to be lowered yet again does not bode well for us at all. And it probably won't be particularly enjoyable for the QC's, either.
But could they possibly have quotas - for errors?

That's a scary proposition alright. Kinda like cops having quotas so they are setting up speed traps and planting stuff in cars to beef up their arrests and fines.

I sincerely hope this is not the case. There seems to be a difference of opinion as to whether they are going to all production or still hourly on this list, so there's that.

Yes, wouldn't it be great if their function, like the old days, was not to punish the MT but to "fill in the blanks" and maybe correct some serial offenders - with words, NOT financially devastating points.

We used to think the words were bad enough - I would take a million verbal/email lashings from the nastiest QA person now for 1 critical error.

I've already been on here and told you what the new changes for auditing were going to be before they happened because so many were apprehensive about it. So don't believe me when I say QC will stay hourly. This board was saying the same thing last year when the idea of QC bonus came up that QC was going to production.
One other thing we in QC do not have any kind if quota to fill for finding mistakes. That is just ridiculous. I could see if you thought QA did that....

With that information, I would not be surprised if they did go to base production pay, with quality and quantity bonuses as the MLSs receive as well. And I am sure it has to do with the minimum wage rate change that is happening in 2015.

on the brought out platforms and all those exciting slides (Nuance LOVES the term exciting!) that showed the metrics to be met for QC positions? Remember also that QC was production based at that time for other QC within the company? Those metrics included productivity per report which takes into account errors found, blanks filled, etc. There was to be incentives put in place based on those metrics, which were "put off" for the time being and the phrase "not at this time" was used. With the new compensation plan coming into play, these metrics will now be instituted. Point blank, hourly QC is just not cost effective. This takes money from the upper layer management bonuses and we all know that is NOT going to take place. Believe what you will well-informed QC, but you might consider putting down that cup of Kool-Aid for just a second and question exactly where your info is coming from. You need to start sipping from the cup of reality.

any change in our pay rates is EVER a positive change for us. Is it ever? No, NEVER. The part about base pay plus "bonuses" or "shift differential" is a crock, because who even gets that now? And how many hoops are you expected to jump through to qualify? You just KNOW that they will try every trick in the book to keep you from qualifying for any extras they claim to be offering, and that the ultimate reality will be that pay will be even lower than it is now. MTs are being financially pried out of the company (in order to save money on lay-offs further down the road). This is just their latest scheme - one of MANY.

I will check for work during my schedule. Work it if possible, if not, just use the no wrk excused absence comment. I am not going to work around the clock to try and make it up. Ridiculous. If you want to, that is okay, but seems crazy to ask people to do this.  Why don't the supervisors/TSMs have a call about this. We get an email, but never anyone to tell us directly about a change. Sounds like they are taking out the LPH requirements? Or am I the only 1 thinking this? Sounds like they take out the requirement for LPH. Now your lines may drop due to focusing more on QA/blanks/rereading,etc.  Normally that would lower your lines and lower your pay. Now it will lower your lines BUT you will send less to Q and have less mistakes so your pay will ultimately stay the same. This is how I am taking all of this. Does anyone know if the PTO we see as of today in teamwork is the correct remaining PTO for the rest of the year?  I was not sure if the pto we used previously in the period ending 12/26 is reflected in the current PTO in teamwork yet.   This is confusing me since we will get a 12/31 paycheck.  I know pto will not accumulate until next year but it looks like to me I did not lose any PTO even since taking a few hours ending the 26th.  any clues out there?  ...

just continue to take what I can get until there is nothing left.  If it means working here and there I guess that is what I will do.  As far as coding, MModal is advertising everywhere for coding people and I can assure you they are training coders and that work will be heading overseas as well so why bother.  They will keep the rates low and there wont be any money in that either.  I guess the only way to even try to survive in transcription is take what you can get unless ...

I got a job with a small local MTSO.  The pay is only fair, and there's no PTO, but there's also no VR and my insurance is free (not for my family though).  The owner and I both know that the days of MT are dwindling but we are going to ride it out till the end.  The beautiful part is her office also does coding and billing.  Not the really indepth inpatient coding, of course, but office stuff.  She knows I want to learn how to do that so when the MT work is go ...

Since 2012, when Nuance "officially" took over the company I was with, they following has been very consistent. 1) Cut pay, then offer some type of carrot (incentive, bonus) to help us "make up" the loss in line rate. QA us to death for any little error, calll it a major error or a critical error to lower our quality score so that we cannot make 'incentive' or 'bonus.' If we do re-read the reports in order to bring quality scores up, our lines go down, and then it is a sum z ...

This probably won't be a very popular thing but, for myself, I'm going to wait and see before I get all bent out of shape about it. I know that Nuance is not famous for doing good things for the MLS, but still, speculating and having us all being paid minimum wage before the fact is not helping us.
