audit the auditors as they have proven themselves not as perfect as they seem to think they are. when they change grade school grammar to something incorrect and change government names to incorrect names, this infurates me. we type it correctly, and they change it to incorrect stuff .... this is JUST NOT RIGHT people.

That is what reversals are for. I copy everyone I can on reversal emails. I put in for the reversal through FIESA and if it's major/critical there is an email that I forward. If not, I make an email and say I want a ruling on this error. That way if it is allowed to 'expire' i.e. no one checked it to see whose error it actually was and so I am stuck with it, I can go back to original person I emailed and others I CC'd on the email and say 'why didn't TSM do something with this?'

The only way the auditors are held to any kind of standards are the reversals. They are supposed to be audited by other auditors, but I wonder if that actually happens or how honest it is. It is a major pain to do the reversals, but there are a few auditors who maybe should be looked at a little closer to see if they are actually qualified. If enough MLS ask for reversals from the same auditor(s) over and over, someone WILL notice.

I get an upset stomach about requesting reversals. I don't do it without sitting on it for a day or so. this week I got dinged with a misplaced comma and for not spelling out the word "rehab." rehab was sited as a common abbreviation that should have been spelled out. really? has anyone ever used "rehab" before? site specifics do not site it as a common abbreviation. any thoughts?

In the transcription guidelines there is a list of acceptable brief forms and rehab is not on it. So what?

I and every other MT use rehab many, many times and it is not incorrect. If there is nothing in your specs against it (or nothing to state they don't want any short forms or abbreviations), you DO have every right to request a reversal.

By all accounts, you really should obtain that reversal AND if not the QA/QC who nitpicked it should be FORCED to put it in the specs, the guidelines, or both if it is against the rules.

Every time this kind of thing happens I request it be added to the specs, yet I have yet to see one thing added, despite them being able to ding you.

I now keep the transcription guidelines AND the account specs open at all times and do a search if anything niggles. I got dinged because I bolded when it was clearly dictated "BOLD ON" because no bolding "unless account specifics instruct to do so" (per transcription guidelines, I had no idea).

I also had an issue with HEENT a while ago. QA were differentiating that they heard HENT and not HEENT so one point off for that. So for quite a while I ended up just blanking HEENT (which looks ridiculous in a document) because I could not hear if it were HENT or HEENT (and I doubt very much whether any doctor ever says HENT anyway because it's not a thing), but now they seem to have stopped doing that. I still got the points off though, no reversal.

If they dock me again for that I will go back to blanking HEENT again. Still nothing added to specs to alert that that is something you can get dinged on.

While we have the right to request a reversal, it seems they have the right to not reverse anything if they don't want to, and they do NOT seem to have to justify their decisions nor clarify them very well IMHO.

Why, it's almost like they don't want you to know the rules so they can get you.

barely scraping by and passing according to MQ? I just received an audit that had a score of 99.3%. I'm told I passed the audit but got a critical error (left out a word) so she will need to do another audit next week. Fine. But how can I get a critical error (in their eyes anyway) and still "pass" the audit? Can somebody tell me what this audit/QA procedure is now? I'm so confused with when they are done, how many reports are done, etc., etc. I had been told earlier that 5 reports would be randomly pulled monthly and checked. You would be notified you had been audited and the results. If you passed, another 5 would be done the next month and so on. If you didn't pass the audit, then a redo audit within 7-14 days would be done on another 5 random reports. ...

I don't understand how auditing works. Two of my reports were reviewed, and in each case, a word I had puit in had a line through it, but then it was replaced with the same exact word! For that, I got marked off, getting only 99.50% or 99.75%. I wish there was a way we could give direct feedback to the auditors at the time we're viewing our reports. ...

I have managed to keep my QA score above 99% every week until last week. Before last week, I normally have 8 to 11 reports pulled for audit, some short, some long. I have only had 3 audits in 2-1/2 years. Even though I received notice of scores of 99.6 or 99.7, I did not receive any account of what my mistakes were to produce those scores. A comma? A hyphen? Okay, I may be the last one to know this, but I thought the only QC audits I was getting were the ones showing up on the DQM QA Feedback screen where it says (1 unread) or (how many ever unread). When you click on them, if the report has been audited by your QC, it will have a percentage score. But that's just for the reports you sent in with a blank. This is silly Not only do we need to follow CP guidelines but BOS and then DocQscribe standards. So even if you have the first 2, the third will get you audit. Why can't they spend the money and create one set of rules to follow and be done with it. I got CRITICAL errors on audit for things like: I typed "I used a translator telephone..." (that is what I heard) and the person auditing changed "a translator telephone" to "the translator telephone" (I suppose she heard "the" instead of "a"). In the physical exam portion of one report, the dictator used "was" throughout except for one time he said "is" and so I changed it to "was" to make it consistent and got a critical error for that. Does it seem like no matter what you do it is not good enough? Here lately it seems like they only audit the worst dictator. The only reports they have audited for me lately have been ESLs lately. I'd like to say I have had to learn accounts on my own and have had to cross reference many, many times. Yes, I have seen blanks where if someone had taken the time to hunt for the word, it would have helped me also, but for the most part with what we have to work with, I have seen words that are highly inaudible by many, many doctors with bad dictation and yet what was typed was a wonder we could make anything out of it. Just wondering if anyone else has had this happen lately. I received a quality audit on 4/25 on my audit, which I passed at 98.3% (had one icky one in there). Then on 4/29 got another email of an audit of 5 reports, did not give a total score, but they were all pretty darn good (3 at 100%, 1 at 98, 1 at 98.75). I am NOT going to read all those posts, but I just wanted to let you know that you are lucky you only get audited once a month. I get audited DAILY, at least once a day. Today I got audited twice, one 100% the other 99.75%. If I can remember correctly, I got dinged for typing something like "Aorta was tortuous" instead of "The aorta was tortuous". I work on a very difficult account as I'm sure many of you do as well, and I spend a great deal of time listening and re-listening to these difficult dictators to make sure I am hearing correctly. I want to repeat this for those who may have missed it and I am not looking for pity. Was moved to a new account after 4 years on 2 very difficult accounts with only 1 or 2 failed audits (and was always given a second chance). After 24 hours on the new account and telling them there was one particulary dictator I could not hear because he kept moving away from the phone, they audited me, included reports from that dictator, and immediately put me on 100% QA at which point I finally ...

I have yet to pass one, how the heck, with say 3 minor errors can you possible pass these things? I want to quit so badly but have no prospective jobs. So the question boils down to basically, is at what percentage of errors are they firing people? 99.5, 99 what? I just cannot see how this is possible to do.
