Do the physicians know what we do? - Just a vent.

During all the eating, nose blowing, just sitting there and reading the chart, talking to the patient and office personnel, and accepting phone calls during their dictation, do they realize there is a person on that dictation line?  Do they even know what we do?

I have a couple doctors who do this constantly!  It is not so bad if I am typing, but on VR all of this chatting and phone calling comes in on their dictation and has to be deleted constantly!!  Frustrating.

The MTSOs should discuss this with their clients, which will never happen.  I would never do this, but if I could write an anonmyous letter to the client, the above is what I would enlighten them on.

I had one yesterday with a heavy sinus drainage who would take a huge sniff and then a loud, audible swallow throughout his dictation. It made me want to gag. Would he have done this in front of his colleagues? Then there's the lengthy phone call to presumably his wife in another language. Regarding the phone calls, often when they resume their dictation, they don't connect it with the sentence they left off with, so we get to figure out what to do with it. I've noticed too, that often they speed up as they are about to wrap up the dictation and get out of there, right at the most important part - the Impression and Plan. I think for most doctors, dictating is a required duty and don't realize at the time that it's also a legal document that could cause them future embarrassment or worse, and I don't think most even consider that there's a human being is on the other end listening to them.

That is the response I got from my team lead when I complained about the radiologists. These rads CONSTANTLY clip their words and my suggestion was that someone in that facility needs to tell them what they're doing incorrectly, maybe show them again how to use the machine... if they are not told, how on earth can it get corrected? I worked in a large imaging center with several satellite offices and had no problem telling the physicians how to use the machine and how to correctly dictate, it's not that difficult. These physicians are people just like us, they are NOT God! I think the MTSOs are scared to speak up because they think they'll get the ax from the physician/facility. Instead, they expect us to be miracle workers and mind readers, which I am not!

I used to work in a facility w/ a couple of horrible docs. Fortunately for me, they had to come outside our office to complete their charts, so I could take them aside and ask them to listen and tell me what they said so I could fill in the blanks. Two would listen and say, "Oh, I dunno, just this in there" and give me something that didn't even resemble the dictated word. They are not gods, and 9 times out of 10, it's the board and CEOs who make the decision to go w/ an MTSO or not, so if we had better dictations that we could send back to them in a correct and more timely fashion if the docs were taught better, wouldn't it benefit the facility?

I used to work for a doctor who was having an affair with his nurse. After the clinic closed, they would enjoy wine and listen to jazz music in his office while giggling and smooching and...yes...dictating. I often wondered if he just didn't get the fact that someone was listening to that. It was so obvious what they were up to.

during residency required class should be Dictation 102. These definitely are legal documents, wonder if the docs even read them when they sign them, but they certainly oftentimes do not know how to use the dictating system or correct methods of dictating - "full stop - period - new paragraph."

I agree, the MTSO is no help with these matters as they evidently think the hospitals will discontinue their service. Sad, ain't it. We all do the very best we can and are putting up with more and more sloppiness as time goes on and get no support from the MTSO we for some unknown reason are still loyal to - but then again, we make the big bucks now, don't we?

When I first started out I worked in a small hospital. You transcribed and did charting. I actually got to know some of the ER doctors because as the new girl I got called there a lot to bring charts and reports. One of them had a mother who put him through school as a transcriptionist. He knew what she did and he told me that's why he purposefully throws new ENGLISH words in there all the time we had to look up to keep us on our toes. He was cognizant of nose blowing and eating, but man sometimes I wanted to smack him spending 20-30 minutes (that's before I went to line per hour rate and was hourly pay) to find some new word of the day. He also spoke VERY FAST, but I find that typical of ER doctors.

I need some help.  I have many, many years in the industry with transcription, VR editing, QA and auditing and I have decided to cut out the middle man (the transcription companies) and solicit physicians on my own for work.  My problem is not knowing the going rates to charge.  I know they are low, but how low.  The transcription companies are paying us very low rates, but how can I find out what the dr's are actually paying, so I dont cause the market to drop ...
