Anyone know? Are doctors and NP or anyone - who dictates medical

encounters, are they required to have a short course on proper dictation procedure? I would think, being that this medical record is sooo freaking important that we are drawn and quartered if not perfect, that these professionals who generate these reports would also be schooled about proper pronunciation, enunciation, etc. before they throw it out there for us to perfect. Getting kind of sick of these lazy, mush-mouthed, slurred, rapid and mispronounced dictations.

I've often wondered about that myself. I think it would benefit everyone if they did have something like that. I'm also tired of lazy careless dictators.

Are you suggesting it be a part of their course requirements for their degree -- required by the school? Or training provided by the hospital/medical facility on their specific program?

Who exactly would enforce this and how?

While I agree they should all be properly trained by the medical facility on their specific program/speech wreck/point-click/whatever -- I don't see it EVER being implemented across the board as a "requirement" for their degree, their license, etc. Sad fact is, the schools don't care - they are focusing on other quite honestly more important areas of education and the medical facilities don't care because they put all the responsibility for a quality document on our shoulders or expect speech wreck to do it. The doctors certainly don't care - they don't have the time or inclination to be trained on something they consider an administration task. The MTSOs have no teeth to tell their clients to train their doctors.

In a dream world this would happen, but not realistic.

When it was in-house, the MR department head would call out doctors when they got sloppy. I remember hearing that from an old-timer.

Ayn posted "MTSOs have no teeth to tell their clients to train their doctors" and that is what it comes down to. We used to train doctors when had communication with them. Since that has been taken from us, no one communicates with the godly doctors for fear of losing the account. Thanks to the MTSOs, they dictate horribly, the mess comes to us to fix and they get back a perfect report. This tells them they are "good dictators" and no changes are needed. Little do they know that erroneous info is being put into their reports, with QA copying what they think they are trying to say from previous report...something that is not easily detected by the doctors quickly going over a report. However, I'm hearing comments about this from doctors about this... "That's strange, his hemoglobin was normal last week...hum" It's going to backfire on them when the malpractice suits start coming in. Then health care admin will look at the problem.

I started transcribing 10 years prior to the existence of MTSOs and docs have been good, bad, and ugly since I started, so I can't buy the 'thanks to the MTSOs' accusation.

MTSOs will never approach the docs, it's pretty much not allowed in in the hospital setting. They can, however, ask the hospital MRec. manager to speak with this one or that with tips. Chances are they won't, but worth a shot.

No one formally "trains." How do you track them down, invite a staff of sometimes hundreds to classes? Here's what they'll say, "I've been dictating for years, I have no time for this nonsense." Think about it, at this point in your career, would you stop and take typing 101? And I believe no one would approach anyone about the art of enunciation aside from a speech therapist.

Mostly, though, my point is that it is not the responsibility of the MTSO. Sell it to Med Schools.

I just don't think it will ever happen for multiple reasons.

The facility I work for has already implemented speech wreck for the clinics. Did not train their doctors one bit, and it is a nightmare. Now, they have purchased speech wreck (albeit a different program - just to confuse everyone from MTs on up to doctors and make everything less compatible - go figure) for the hospital side. I was included in a meeting with the hospital, IT, HIM department, and 2 vendors trying to sell their speech wreck. One was the vendor that the clinics use. They asked - what is the accuracy % "out of the box" (i.e., no training, sit down and start using it) and the vendor claimed over 90%. Most in the room had the sense to know this was not true and said so. Vendor claimed this would be true IF physicians were trained how to use it and not just handed a microphone and told to go. NOW - Do I believe that? NO. But, I do believe it would certainly work better if the doctors were trained just in basics if nothing else. For example -- You can no longer say "go back up and where I said this now insert that" We all know what speech wreck is going to type because we deal with it every day, and we know how long it takes to reformat, cut-paste, and correct the mess that speech wreck spits up. The doctors do not know or realize how speech wreck changes OUR jobs. But, is that their concern or their problem to fix??

I guess that's the question -- do we expect the dictators to be trained and made to change how they do their job just in order to make our jobs easier?

I really don't expect the doctors to be responsible for "fixing" how speech wreck works. (Although, honestly, I think the youngers ones coming up are more tech savvy and will do much better with EMR/speech wreck/point-click than the older generations who are set in their ways.)

I just want to be paid a fair wage for learning new job skills while maintaining accuracy and speed while now being an editor instead of a transcriptionist. I want the MTSOs and the hospital administrators to acknowlege that this is an issue (dictation practices/styles affecting our work) and take it into consideration instead of thinking it will magically solve itself.

I've always thought our job is kind of schizoid and wonder if it really is all that important.

On the one hand, we're told how vitally important our job is for patient care, etc. -- on the other hand, no one can be expected to ask the big, important doctors to maybe try to enunciate words a little clearer or slow down just a bit 'cause it sure would help us out with our piddly little jobs -- but I guess our jobs really aren't that important.

My paycheck does not reflect the importance of my job either.

seriously--i don't think they have any idea what they sound like, or what we hear.

i worked in-house, and problem docs got training in correcting bad habits.

i honestly don't understand why MTSOs don't tell these people. no need to be rude--just explain the problem. or just send reports back with [unintelligble] marked where what the doc or whoever is talking over the screaming kid or paper shuffling.

enunciate clearly when dictating. I don't think of it as just better to make my job easier, but for the overall correctness of the final patient report. Doctors, after all, should want what is best for their patient and an accurate report is best and starts with a clear and accurate dictation.

Doesn't it drive you nuts when a doctor consistently cuts himself off when he dictates?  I sometimes get half words, the last couple letters of words, places where I haves to leave blanks because they say something like:  EKG showed (silence) rhythm.  What kind of regular?  Regular rate and....?  Arrrrggggghhhh! To the poster who posted below looking for information on TTD.  They don't have their own accounts.  They work for Superior Global and maybe others.  Superior pays them to find ICs.  They probably pay them 4 cpl and then TTD pays you a cruddy 3 cpl to edit VR reports.  They don't offer direct deposit because Superior is paying them and then they pay you, which is why you have to wait a month for your check.   I would never work for a company that is just bascially an agency hir ...

that they are dictating for ASR and how to do it, punctuation and all. They need to take some responsibility for this. If they would dictate for ASR, ASR output would likely be better. I base this on the doctors who do dictate everything, including punctuation and the fact the output is good for this doctors. Although, these doctors do tend to speak in a more understandable manner. Ok, I'm curious, but can doctos do their dictations anywhere? A lot of times, I hear a bunch of doctors in like some sort of dictation room. You can hear other doctors doing dications. My girlfriend told her internist about our plight and the doctor said that they were actually forbidden to use a transcription service and to do it themselves instead.  She hates it and would rather have a transcriptionist do it because they are so good at what they do!   I don't understand why they would be forbidden to use a transcriptionist.
