I made a longitudinal oriented incision and expressed some purulence out with pressure, cephalad and caudad, knee-v-eel, and lateral.

The patient has a infected brown recluse bite on his anterior proximal lateral knee.  Any ideas?

like you have with cephalad and caudad:


cephalad and caudad, medial and lateral.


Not part of the question, but I would also hyphenate longitudinal-oriented .. as it is modifying 'incision.'

Good example of stepping back and looking at - the context.

People bump into a tree in the forest and get too caught up in taking out their magnifying glass and examining that one tree over and over. Of course, it rarely sounds any more clear the second or the hundredth time you listen to it, and while they're doing this they completely lose track of the sense or meaning of what they've just been transcribing.

This is "word-for-word" transcribing instead of "contextual" transcribing, and it's typical of beginners although we all can fall into the trap.

Here we have a term nestled in a sequence of directional terms. Step back, look at what the doctor is TALKING ABOUT (as opposed to what he is SAYING), and the context makes it crystal clear what this word is almost certain to be.

When you hit a blank, stop listening to it over and over. Step back. What's he talking about? What's the context? Grab ahold of the sense of the situation again, instead of a mere sequence of words. That term you can't discern fits into the logic of the sentence. If you lose the sense of what's going on - the story that's being told - you're stuck with word-for-word transcription, and that's a guaranteed loser for you.

Dictated:   Minimal s/l sc____ing in/of Hoffa’s fat pad as well as some degenerative change of the distal femur anterior to the weight bearing surface was noted.   I first thought it might be "scarring" but he is not making an "r" sound in the middle .. more like "l" or "w" - sculling, scowing.  I am not familiar with this term/phrase in MRI.  Is anyone else?  Is it scarring ? or something else? and then would it be "in" or "of" Hoffa's fat pad?
