sentence drug help? I have a new doctor, and I do not know how to put her sentences.. - dawn

Continue Sinemet tablet 25 mg to 100 mg orally 360 one tab q.i.d. 90 refills 3.

 

she actually says "25 mg dash 100 mg" but I put the "to" instead of "-"

 

I am not sure how to punctuate that sentence or break it up if needed?

It's a combination drug, so the doctor really wants the "dash". (You can also use a slash.)

I think I would type: Continue Sinemet tablet 25-100 mg orally. #360. One tab q.i.d. (90) Refills 3.

 I was just wondering if anybody out there knows how to switch back and forth between capitalizing the first letter of sentences and not capitalizing.  This is a problem when I go to type H. pylori or C. difficile, because it keeps capitalizing the "Pylori" and "Difficile".  Of course, I asked Support, but they said there was no way to change it.  I just wondered how you guys handle this.  Appreciate any help!! ...

I am transcribing a list of drugs in a report, and it sounds like the doctor is saying "Verelan SR." I can't seem to find a listing for Verelan SR anywhere. Could he be saying the name of another drug? Is the SR part nonexistent for Verelan now? This is the context: "For hypertension, she is on Avapro 300 mg daily, Verelan (SR) 240 mg daily, and Lasix 80 mg daily. Her blood pressures are generally under 130/80." This patient has type 2 diabetes, if that helps any. ...

I'm transcribing a dictation by an Asian doctor and I'm unsure what to do with this sentence. I'm pretty sure it is exactly what he is saying.  "...drainage from her right ear 2-3 days ago. unsure of punctuation and also the last sentence. I typed all as dictated, but think there should be a comma after femur, and no fractures noted as a separate sentence.
