New Dr.  He sound congested so I am struggling with this op report.

Due to the inflammation I was not absolutely certain that this was extrasphincteric and so an adequate drainage procedure was done by taking a quater-sized divot out of the skin down to the abscess cavity to "breeding the necrotic tissue".

A little research reveals that it is of the utmost importance to debride necrotic tissue.

Further, bleeding denotes viable tissue, not necrotic tissue.

To address your final point, we believe she is hearing "debriding" rather than "to bleeding"

You must have not understood my post. It's "debriding" NOT "to debriding". Thus: "...by taking a quater-sized divot out of the skin down to the abscess cavity, debriding the necrotic tissue".

Debriding ALLOWS for adequate drainage. Kindly note the following reference:

"Debridement is the removal of foreign matter and necrotic tissue from a wound.Edmonds gave the following rationale for debridement of ulcers: debridement enables the true dimensions of the ulcer to be perceived, and allows drainage of exudate and removal of dead tissue, both of which reduce the risk for infection."

This could be argued until our computers turn to dust.  The problem is no one, except the original poster, can HEAR what was said.  Either could be correct.  No one knows for sure what the dictator’s intent was.  We can only speculate.  


But, in playing devil’s advocate, isn’t the conventional wisdom that you go with your first instinct?  


The original sounds like was:  to "breeding the necrotic tissue".


But the followup “figured it out” was:  bleeding and necrotic tissue


To my mind, there is a big difference in the sound of “the” and “and” – most people do not have trouble distinguishing the two.  So, which did the original poster actually hear?   Only they know.


My rationale for both scenarios:


If, indeed the word “the” is clear as a bell, then it would be more logical to go with:


Scenario 1:


1. adequate drainage procedure was done by taking a quarter-sized divot out of the skin down to the abscess cavity "debriding the necrotic tissue".


The “to” before breeding could easily be misheard.  So “to breeding” could be “debriding.”  Also, the first part could be a complete statement, with an additional statement:


adequate drainage procedure was done by taking a quarter-sized divot out of the skin down to the abscess cavity (that could be perceived as a complete thought, with the doctor then adding a statement and not including the word “and”) debriding the necrotic tissue.


Adequate drainage procedure was done by taking a quarter-sized divot out of the skin down to the abscess cavity; debriding the necrotic tissue.


Scenario 2:


If, however, on further listen, the original poster is positive they hear the words “to” “and,” then the second scenario would be more likely.

 Bottom line:  Only the original poster knows what they heard and only they would be able to stop this argument.  


It all comes down to whether it is “the” or “and”
