please help with phrase - confused

When a physician dictates “It should be noted as well within the family history” and you are already typing the family history. Do you still type “It should be noted as well”? The doctor dictates this frequently.

Sometimes when they say "as well", it's not so much to say "also", but rather to place emphasis on a finding that may be pertinent.

It's a matter of the doctor's style and expression. My vote is: Don't change a thing. :)

it just does not make any sense whatsoever or does not fit in with what was previously said. DO NOT CHANGE a dictator's dictation just because you think it is not needed.
