I would probably put: grade 1 one-column
But that is because of my accounts. The QAs HATE commas and BOS says not to put two arabic numerals next to each other. You may have to adjust depending on your client preferences.

Grading System:
Size varices: A sm, B med, C lg
Number of columns: A 1-2, B 2-3, C 3+
Red wale marks: A none, B mild, C severe
Cherry red: A none, B mild, C severe
Risk of bleeding:
Grade 1 AAAA = rare
Grade 2 BAAA = unlikely
Grade 3 CBAB = possible
Grade 4 CCCC = likely
