Why did QA correct this? - Anon

Posted: Apr 21, 2011

I sent in a report with 2 blanks because of a discrepancy.  When I got the feedback, every single time I typed "diabetes type 2" as was dictated, it was corrected to "non-insulin-dependent diabetes type 2."  If that's how they want it, it's okay with me, but it seems a tad like padding the line count!

What??? - Jax

Wow, that is padding and if not what the dictator stated (unless in client profile) completely wrong!!!!!

You can have type 2 diabetes AND be insulin-dependent - no name

I assume they looked at the medication list (which we hope was complete for their sake) and took this upon themselves? I would be in huge trouble if I ADDED words not dictated to a report and rightly so. This is completely unacceptable, QA or not.

You got that right! - nm

I am a type 2 diabetic ..sm - nowwhat

[ In Reply To ..]
Someone does not know anything about the medical field. There are thousands of type 2 diabetics who must go on insulin! I am one of those. I am on sliding-scale 4-5 times a day and twice a day long acting insulin. Sounds like an ILP QA who are supposed to be soooo much better and smarter than us. Were u able to listen to the report again, and just missed hearing the non-insulin dependent part? Or even more, did the medication list show insulin and it should have been insulin-dependent? I am not fussing, but these are the types of things that we all have made mistakes on by not being able to hear correctly.
oh, I was NOT putting down the OP. sm - nowwhat

I guess when one starts typing replies, our minds are thinking faster than our fingers. I mean this to be that a QA is supposed to be there to help us, and I think there are good ones out there. But changing something this critical is so wrong. OP you did the right thing by sending it to QA, QA was the one wrong in first not explaining to you that they heard non-insulin and hopefully they also questioned if there were insulin medications listed. The OP was not the one in error, IMHO. Like the poster below, speak up or forever lose your peace..

Was it changed with a feedback marker? - New MEE

[ In Reply To ..]
or was it changed by just editing the text? Using feedback markers turn the changes a different color so you can see them. If they just changed the text, you wouldn't notice unless you remembered what you typed. In any case, if they used a marker, they do not get credit for what they typed. I found this out the hard way. I was once sent a job to QA that was 5 minutes long and literally had only 2 words typed in it. I typed the whole report within a feedback marker (probably 100 lines worth) and got credit for only 1. I'm not sure if we get credit if we just edit the text, because I like to let the MT/ME know of the changes I make.

Either way, there was really no reason to change what they did. Especially without giving a reason. I've only been an MEE for 6 months, but a transcriptionist here for 10 years, and I remember getting dinged for really stupid stuff, so I ask myself if any changes I make would tick me off when I was an MT; if so, I better give a good reason for changing it in feedback.

go ahead and ask them why they changed it - sm

I agree that it's inappropriate for them to have made that change. It's also unethical and they are overstepping their bounds.

I assume they think they have to expand diabetes type 2 to include NIDDM in the name, but in that case, why didn't they throw in "adult onset" too?

Inappropriate. I would ask them why.

I would send an email to your CCM to let her know the correction, it was very inappropriate for the QA to correct that.
