Tranpositional capital fragment osteotomy in rectus alignment with good sesamoid positioning noted. The s/l "hyroid" demonstrates no evidence of displacement.

hyoid - as in hyoid bone. Not sure what type of report this is though?

I suspect that it is something to do with the hardware. It is probably something simple that he has just slurred until it is unrecognizable.
I have a dr. who consistently says all capital letters for that. I always thought it was just scarf because it had to do with the type of incision. ...

Is, "Delivered the infant over a midline incision" a sentence fragment because "I" is left out? I am on a verbatim account, but we are not to use sentence fragments. Dictator clearly does not say "I," and I am wondering whether I should add that in? I have only been working for a month and am feeling a little confused now. I was docked some points last week for leaving the I in this type of report out, but yesterday I was not docked, so now I am feeling helpless. Thank you! ...

I understand having a good QA team, but that does not mean picking apart every comma, typo, and, it, and but particularly when their own product "Dragonspeak" a Nuance product being used by doctors in their own offices makes the most shocking of errors and those errors are going directly into the patient's charts without being fixed.
