Numbers and meds - Viking fan devastation

I know we should type the sentence below as:

Advair 250/50 1 puff twice a day but to me, that looks like it could be confusing when the dosage and amount of med are so close together.  I know we're not supposed to do it like this but this is what I type instead:

Advair 250/50 one puff twice a day.

Am I committing a horrible crime here?  Undecided

I haven't heard anything myself about it. I don't care what the BOS says - if it looks confusing, I'd rather be written up or reprimanded for typing it the wrong way than risk having a patient get a wrong med dose. And I can't stand when doctors say things like "This is a patient that underwent...." I change "that" to "who". People are not objects.

after medication amounts so we use 250/50 mcg 1 puff twice a day. But you are correct, you DO NOT type two numbers in arabic form consecutively, so it would be 250/50 one puff if typed that way.

The second way you wrote it is always how I type it for all my accounts (except when I did hospital) for the very reason you state. Even when it is an oral medication, I always type out the number, i.e. 5 mg two tablets once a day. Unless the account wants the numeral, I will type it out.

Does Everybody Keep A Log Of At Least Job Numbers?

I've been keeping a Notepad document running of all my job numbers in order to confirm Fiesa feedback (sometimes they get the wrong MT, had that happen 3 times so far, so you need to check that if a correction is not familiar to you) or whatever issues might come up. But occasionally I forget to even log a report, and I realized that I can double-check via Metrics as well as with my ID and date on Search Dictations, so why bother?  Just wondering if some don't bother with this and ...

Doc dictates 2 meds for pt - s/l "care-dih-pull all" and "pro-mid-per-all".  Pt has BPH and Parkinson's.  I thought carbidopa, but this doc is pretty clear usually and it definitely does not sound close.  They both end in the "all" sound. Ideas? ...

One med is running together - s/l either acon, toprolol (? she means Toprol), or acontoprolol.  Either way, I'm not able to come anywhere near to finding the one that s/l acon or akon. The other s/l pro keratine XL.  I can find something on the web called Pro Keratin XL, but not sure about it. Thanks! ...

I just put some things together that are enlightening.  MQ/Spheris has 12,000 MT's in the U.S., not counting those overseas.  Transcend has 5,000 MT's in the U.S. and another 1500 in India.  Webmedx has 1200 MT's.  KeyStrokes has 600 MT's.  TT has 200 MT's.  Softscript has 200 MTs.  NEMT has 200 MT's.  Transolutions, or whatever they are called now, has 300 MT's.  Medware, again not sure what their new name is, has 3 ...

My actual ASR lph and what they say I could do if there wasn't any "inactive" time is usually almost double. I have no idea how I can humanly close that gap. And yes, I am not using the dreaded mouse. I put my nose to the grind stone with absolutely no inactivity due to doing anything other than edit, and yet, I still come up with the lph they report I am doing rather than what they think I should be able to do without any inactivity. First off, is "med" an appropriate abbreviation or if I need to expand it.  Also, I'm having trouble with a few of the medications she's listing.  The first one sounds like Serapril...I know there's Seroquel, and that's a possibility, but it really sounds like she's saying, "pril." Is there anything else that sounds like that? The next one s/l permecidene and the last one s/l kolite. Any help would be greatly appreciated! Thanks so much! ...
