The doctor states the patient takes NPH 20 two  b.i.d. - would that be transcribed that way-or would it be 20 mg? -it seems like the 20 should have some unit next to it for clarificatiion

Not only transcriber, a diabetic here, and I cannot make that sentence right in terms of dosing, it really should be "NPH 20 units b.i.d., as that is a correct way to give NPH, but could the extra "two" that you are hearing be the doctor starting to say "two times daily" and he switched to "b.i.d."? That is a about all I can think of or reference, hope this helps!

Are you sure you're not hearing "NPH 22 b.i.d."?

Yes, there SHOULD be a defining unit following the *22* -- and it most definitely would NOT be *mg* -- but it's entirely possible that the doc assumes YOU know how insulin is dosed -- UNITS -- and didn't take the time to be specific because it's universally understood. (It wouldn't be the first time a doc has left off *mg* or *mcg* or *units* on a medication list.)

And you would type it exactly as dictated, or flag it. Please don't take it upon yourself to correct any medication to what you *think* it should be.

I believe insulin (any kind) is dosed according to body weight: units/kg


It comes as an injectable liquid, not a fixed-dose tab or cap. ;-)

Don't know if this is helpful, but I was a gestational diabetic who took 14-16 units of NPH before meals and 22-24 units at bedtime. I believe 22 units could be a proper dose for a patient but as mentioned, best to ask to make sure -

I have slowed it way down and sped up, and it is clearly lan set.  Not Lantus.  I have archived lancet insulin, and everyone says it must be "Lantus" on those questions, because Lancet is the blood stick checker.  So...I am wondering if there is a "lancet" device that delivers insulin that I am missing, or do you think this is a flag? MEDICATIONS: Humalog, hydrochlorothiazide, Lancet insulin, Plavix, metoprolol, and lisinopril.  ...
