Is the correct way to type.....DiaBeta five milligrams one every a.m.  OR DiaBeta 5 mg 1 q.a.m.  OR something completely different.

I only have q.d. and q.o.d. in my list for dangerous abbreviations and have that q.a.m. and q.p.m. are okay.

I guess it could be different for different companies, but that should be in your account specs. Hope this helps!
