Based on my experience, you are not allowed to add or delete anything on a verbatim account.

mg instead of mcg, and I was blamed for not having it changed. It was a verbatim account.
So, whatever we do, it's our fault!

No, I don't even add this on an account that has leeway with editing. It's up to them to say it. HOWEVER, if he or she says mg on a drug that is mcg, that's another issue. On a really strict verbatim account would probably flag it per appropriate protocol. On an acct with more leeway, I would correct the mg to mcg.

We are here to check for spelling and grammar, what else? The drug units? Do we also have to point out a surgeon's mistakes during an operation?

If you are not interested in producing an accurate medical report, find some other profession. Spelling and grammar is certainly less important than drug dosage and surgical errors.
It is the duty of an MT to correct "surgical" errors?" - haha!

For example, "Norvasc 5 at bedtime." Never gives mg/mcg/etc. I leave it just as he dictated it since it's a verbatim account.

Along with the other poster here, I do flag things that are obviously incorrect. I was once editing a report where the physician dictated, "OxyContin 500 mg p.o. p.r.n. pain." I left the dosage blank and flagged it for QA, pointing out that this dosage would be lethal and thus was obviously a dictation error. I've also caught where the physician dictates the patient has an allergy to a medication then turns around and dictates the patient is on this medication in the medication list; these I also flag for QA.

At the end of the day, it's my initials on the bottom of those reports and I wouldn't be able to live with my conscience if I knowingly sent through something to the client that I knew darned good and well contained a glaring (and potentially deadly) medication error.

the doctor is dead wrong about his anatomy? The patient has a problem with his foot and leg, but he dictates ulna. For 4-numeral drug dosages, are commas used in the number?  As in:  1,500 mg vs 1500 mg.  I have the online BOS but have an awful time finding what I need.  (If anyone has any good search tips for that thing, that would be much appreciated as well.)    : ) ...

Is it me or what - I was always told to follow account spec and make sure I didn't use the forbidden JCAHO abbreviations. Lately if I look at an old report to get a slurred/muffled word that I can't make heads or tails out of, I've noticed that others on my same account don't follow the specs at all! The dictator clearly dictated a word with an apostrophy following it.  The word actually has an accent mark following the last letter, but he specifically dictated an apostrophy.  I got it marked as an error. The reason I ask is because this morning I'm doing an acute care progress note where the dictator says "The patient has end-stage chronic obstructive pulmonary disease and she is still sounding pretty crappy and congested."  I absolutely refuse to use the word "crappy" in a medical document if it is not within the context of a direct quote from the patient, i.e. "I feel pretty crappy..."  So I decided to omit crappy and just say "she is still sounding pretty congested."  QA w ...

Hi all, it is late and I am getting tired.  I have a doctor stating the following, "He is placed on clindamycin 10 mg/kg per 5 mL every 12 hours. This is a 75 mg/5 mL solution."  The patient is a child.  I googled and can find bits and pieces of what I input, but nothing all together.  I guess what I am questioning is, do you think he meant to say the first 5 mL, or do you think he was stumbling over it?  Also, does the dosage seem correct to you?    ...

This is almost/sort of a poll. I need to know what verbatim means to you in VR. Do you change any wording? My understanding of verbatim is word-for-word (Merriam-Webster dictionary) unless a sentence does not have proper structure; if the doctor rambles on and puts the word 'and' between words where there should be a period, etc.  (Not included in this is changing mg to mcg in a wrong dosage, changing the medication itself if wrong, or other major medical erros&nbs ...
