Transcribed as dictated on verbatim account - but got it marked as error

The dictator clearly dictated a word with an apostrophy following it.  The word actually has an accent mark following the last letter, but he specifically dictated an apostrophy.  I got it marked as an error.

I don't get it -- it was a verbatim account -- I did what he said - and now it's error.

Sorry this happened to you, but unfortunately, we can't just trust that the doctors are correct with spelling or punctuation. We have to verify everything.

I have corrected gross grammatical errors including verb tenses on verbatim accounts and have been penalized for doing so. I think the OP meant that they knew this was an error but did it verbatim...you can't win with this company.

Verbatim means type what is dictated but not if something is spelled incorrectly. If it was a drug name that the doctor misspelled, you are supposed to look it up and type the correct spelling. Also, if they dictate random punctuation (like some love to do), you have to correct it. I know it sucks. I had to learn the hard way too what "verbatim" actually means.

I'm honestly not arguing with you and I agree wholeheartedly with you. However, QA has repeatedly marked me off for making these corrections (correcting punctuation, verb tense). I HAVE also corrected spelling errors and I have NOT been marked off for that. Nonetheless, QA is so terrible here that honestly I don't know what to do. Actually, I do know...I quit lol!!

When I worked for Transcend, as I recall, there were symbols that we could not use in BeyondTXT (the original), and I believe that included accent marks and anything else that couldn't be found on the keyboard, plus symbols like # and %. So one would either have to leave it out, or use an apostrophe. I'd challenge it if I were you.

the doctor is dead wrong about his anatomy? The patient has a problem with his foot and leg, but he dictates ulna. I wouldn't give it a second thought to leaving it blank, but I already got notice about documents with blanks (usually 1-2 blanks if any, and anymore than 3, to QA). Is it me or what - I was always told to follow account spec and make sure I didn't use the forbidden JCAHO abbreviations. Lately if I look at an old report to get a slurred/muffled word that I can't make heads or tails out of, I've noticed that others on my same account don't follow the specs at all! The reason I ask is because this morning I'm doing an acute care progress note where the dictator says "The patient has end-stage chronic obstructive pulmonary disease and she is still sounding pretty crappy and congested."  I absolutely refuse to use the word "crappy" in a medical document if it is not within the context of a direct quote from the patient, i.e. "I feel pretty crappy..."  So I decided to omit crappy and just say "she is still sounding pretty congested."  QA w ...

I was a transcriptionist for 30 years. I have been away from it the last 4 years, except I am a full time online MT student at an approved school through AHDI. I'm due to graduate in July and get my RMT, then I want to get my CMT. I can't find a job transcribing! Does anyone have any suggestions? Everyone seems to want someone with "recent" experience. I haven't forgotten much after transcribing 30 years and I need a job! Help, please! ...

A friend of mine who works in a medical office told me this afternoon that they've started getting records that are transcribed overseas. She said the records are done so badly that they can't use them, can't trust that the information in them is correct. When I told her my job may go away soon & be sent to India, she said "it can't!" My employer has recently gone from straight transcription to VR (using MedQuist's DocQSpeech). At this point, after 4 months on VR, production is up 11% overall.  The employer says they believe it will go up 48% overall (60% for VR lines, no change of course for transcribed lines, 80% of lines VR). At this point, indeed, at least 80% of lines are VR.  (The quality of the VR output, however, is pretty bad.) We're union, and I'm one of the MTs participating in the wage ...

This is EXACTLY how it was dictated: I cleansed the skin with alcohol from her prior visit (LONG PAUSE) I do remember patient having a very low pain tolerance. That's pretty unsanitary to use alcohol from the prior visit, don't ya think? LOL ...

How is Nuance able to legally justify paying only 88% of typed lines?  I am really curious if anyone has any indea how that can be done.  They are obviously trying to get transriptionists to leave in significant numbers.  There must be some motive we are not seeing.  This must be some attempt to get transcriptionists to leave in large quantities.  What other explanation could there possibly be? ...

This is almost/sort of a poll. I need to know what verbatim means to you in VR. Do you change any wording? My understanding of verbatim is word-for-word (Merriam-Webster dictionary) unless a sentence does not have proper structure; if the doctor rambles on and puts the word 'and' between words where there should be a period, etc.  (Not included in this is changing mg to mcg in a wrong dosage, changing the medication itself if wrong, or other major medical erros&nbs ...
