OK.....
It's an orthopedic chart note. Patient has a chief complaint of hip pain.
Dictated: He has a negative [s/l fair, fairs, varus?] sign, a negative fabere sign.
It doesn't sound like fadir; I'm not hearing that "d." I already had to leave two medication blanks and hate to leave another one for an orthopedic sign that I feel like I should know!
Thank you in advance for your help and ideas!

Hello,
I would like to transcribe from home again. However, I HAVE to get up and walk for a few minutes at least once an hour. You see, I had to have 2 back surgeries in the last 18 months. I've been a transcriptionist for many years and this is my profession. Does anyone have a suggestion what to tell these companies that will hire you to work from home when you have to get up so much? The last company I tried had a "pop-up" box that appeared on my screen if you were up longer than 10 min ...
Any information on Hypertype? Good, bad, and ugly, pay scale, platform? TIA. I have passed the test which I took a little more than a week ago. They want to set up a phone interview but I want to know upfront what current ICs think. Are they flexible. Do you have a schedule. Saw through the archives that the pay is low but how low is low? TIA. ...

The company and employees make all the arrangements, including sending a cleaning service the day before! It's great. The other evening at the grocery store, I picked up the newest issue of Employment Guide. You've probably seen it - it's usually by the doors to the store, and it's a nationwide publication. It used to list actual job openings for the local area. Now, though, it seems to be mainly an ad newspaper for schools offering career training. Anyway, what struck me about this issue is that none of the schools that have always advertised in this paper list medical transc ...
I had an interview last week with a big national company, recruiter was unprepared, kept making excuses as to why and then was supposed to send more info, but that didn't happen. Then was supposed to call again today, no call. I'm going to take that as a sign that this is not a good place to work. Oh Lordy, I really need a job! Any advice please. Just email me. ...
From all of the negativity about Nuance, I was very leery about accepting a position with them, but finances prevail. Today was the final day to submit my paperwork, with training to begin on Monday, and I wake up this morning to find my computer died (I'm on a laptop now). Coincidence or warning? ...

I am doing an H&P and the doctor says:
"Alert and oriented x 3. No sign(s) of (s/l) sedation. No acute distress. Normal gait."
Have you ever seen "no sign of sedation" in an H&P for a cervical epidural steroid injection?
Any idea what else it could be?
Thanks. ...
