I was just expressing to a regional management-level employee at my company how I thought the VR product was second-rate because to make any lines at all you kind of have to blaze through it and not make it a high-quality grammatical document as you do when typing.  She said ya know what?  Nobody cares any more.  The doctors don't care, the hospitals/clinics don't care.  All they want is fast and cheap.  Is it true?  I can't bear to see all this cumulative skill I see on this board just sold down the river.

That's correct. When we went to VR our employer had to really relax a lot of its requirements, punctuation (unless it changes the meaning), if they dictate COPD and it comes out expanded, leave it as is. Maybe they are dumbing this down so that a doctor can dictate and edit his own work, cuz do you think he/she would care about where the period is, the comma is, a run-on sentence. Unfortunate indeed.

Actually, in retrospect, I think the overall decline in quality in MT started when we first went from an hourly wage to being paid per line. That was just the beginning, and it has just gotten progressively worse.

Nobody cares indeed; I really doubt that most docs even read their reports before signing them. This account I'm working on right now has a dozen or so docs, and only 1 reads his reports to my knowledge.

I have to correct every darn think that VR changes, every period, every comma, everything. If the doctor says "The patient came in with..." and the VR puts "Patient came in with..." I have to stop and add the "The" and change the capital P in patient! It slows me down so much when every single line needs to have at least 2-3 edits.

I was told by a supervisor years ago that "trained dogs could do our jobs". That was in the 1990s now trained computers are doing our jobs. Since most malpractice lawsuits from what little I know are settled out of court. They don't care. Cheap is good. Unfortunately you get what you pay for! Better check your medical records folks when you go to the doctors.

I care about quality and go above and beyond. I also have more work on my desk than you can imagine and have had to turn away clients. If you are working for a large hospital, the physicians really do not have the opportunity to give feedback, but they certainly do as an independent contractor working directly with them, and they notice even the smallest things, from fixing their misspelled words to extra formatting done for them--and they appreciate it.

Amen. Thank you. I just received a call from a surgeon today who asked me to take over his office transcription as well and is willing to pay even more than what he is paying now for quality work, so they do care. Thank goodness.

fits if the rare report came back with a typo, even though typos and major errors rarely happened. We also prided ourselves on catching doctor errors and they were grateful. Since big MTSO took over, the docs let a lot slide. Their higher ups told them to put up and shut up.

This seems to be the case where I work. I don't work on line. I saw a report the other day with C6-T1. That can't be possible, can it unless your missing a vertebra? That seems to be what our country is all about now. Just look at the politicians, seems to be all about greed and money. I am with you completely. It makes me very sad. Sloppy and fast technology seems to be replacing accuracy and patient care. Where I work, I think there still are some doctors who care and some that have just accepted this as the norm! Some of you probably aren't even old enough to remember when doctors really cared about their patients. I believe they still would if they were making the rules, but they aren't.

Regarding whether anyone cares about proper punctuation, it depends on the account. Some care very much. Some would rather you wouldn't bother because it slows you down.

I work directly for a hospital and part-time for a hospital clinic. Have you seen the reports self-generated by the attendings, the residents, the nurse practitioners, the nurses? I shudder when I see them. How does it pass JCAHO? We are not allowed to use those abbreviations when we are transcribing. How about "exacbation" or the other atrocious spellings? How do they get away with it, and no I do not think any of them care.

I've talked to doctors who have told me they are so discouraged about the changes that they want to get out of medicine entirely, and will as soon as they can. Others say, it is what it is. They have to deal with it, but don't like it. I spoke to one who loves the changes! It just so happens that he is in charge of them in his large hospital. I'm disgusted with him right now, although I guess it was his job and he did it and is proud of the work he did.

My ENT doc's group practice (he is the head guy) went to point and click about 4 years ago. Every time I went in there, he gleefully told me "your job is going away"! (A very monosyllabic communication from such an erudite man.) He seemed to enjoy "playing" with the new "toy."

Anyway, the point is that some of them feel "special" to be on the cutting edge of whatever new gadget or technology comes down the pike. They think it makes them "smarter" or "superior" or something to have the latest technology.

That's why I argue with all the MTSO shills on this board who keep insisting that the docs will tire of this and go back to using MTs. NOT gonna happen, especially with the cash incentives waved under their noses and the deadline, both imposed by the feds. (You can thank them for destroying MT.)

Anyone who takes glee or feels smug about the state of our jobs, hopefully will lose their own livelihood (or at the very least karma will pay them or those dear to them a not-so-pleasant visit in the future.) Our industry is in its current dismal state not because of MT deficiency but because of corrupt greedy MTSOs. Experienced MTs not only have kept current with their OWN skill set, but also have learned to constantly ''roll with the punches'' and adapt to constantly changing software, VR, and computer technology and anyone who claims otherwise is either a liar or woefully ignorant. Hope your less-than-compassionate doctor loses his practice and let's see how ''gleeful'' he is then.

Thanks for that! However, he isn't likely to lose his patients because that group is the top ENT group in my city. Part of his attitude may be explained by the fact that he has an MBA as well as an MD.

Another thought -- I wish patients would start to complain about their visit time being consumed by their doctor sitting there "typing" at the computer! Not that it would make any difference, but still I would like to see it.

Don't you hate it when you get an email about teamwork and how Nuance strives for excellence and how we're all part of a team? If they strive for excellence, they should pay for excellence. If we're all part of a team for Nuance excellence, then MTs should be paid as if they contributed to the team's excellence. I have recently started with a company that has been "somewhat bashed here."  However,  it is straight typing and if get "mush mouth" repeatedly, I can call  or email and they will change my pool.  I can talk to the VP, and she LISTENS to what I have to say.  I got a call today from tech support and it was to the effect of "what can we do to make your job easier?" I just got my second DEP ASR Productivity Results.  If the person that has time to count mouse clicks, time spent away from keyboard, etc is reading this, then answer this, why don't you take into account time spent reading multiple client profiles, time spent reading sample reports to get information, etc?  Do you not expect us to leave our chairs occasionally?  I personally am more suited to using my mouse.  A lot of the shortcuts just didn't work as well for me.& ...
