Nurse's or nurses' notes which is correct? - melinda

Is it always correct to use nurses' notes in dictation? My reasoning is that it is yes because there could be more than one nurse charting on the case.
I've wondered that too ... - anon

I come across it a lot. I have to admit to using nurses' notes just for the reason you stated above.
I put nurses' - Just in case

Nurses do have very long shifts, so it's hard to know when the doc saw the patient in relation to each nursing
shift, but I figure nurses' is safer.
Also, there may be an RN as well as an LPN on the same shift, and they both take vitals, etc.,so...
I have to say I like better when they say "nursing notes."
Depends on the account - ss

I have to do it both ways depending on which account I am on at the time.

'how many nurses does it take to screw in a light bulb?' I don't know... so maybe multiple nurses filled out the chart or one nurse did...its up to qa to decide how much they like you when they ding the note.
nurses' notes is correct - multiple nurses

If you've ever seen nurses' notes, you know it's more than one nurse writing them.
I started taking the nursing program at my local college 10 years ago and loved it, but dropped out after a year to get married, bad decision! So, just looking to see if anyone here has been a nurse or is going into nursing and what they like/dislike about their jobs. I am also looking into the RN program in the future. ...
Some people are in US are worried about offshoring medical transcription job to other countries like India as their medical records would be misused or it is under wrong hands. Then what about immigrant doctors and nurses working in USA? There are lacs of doctors from different nations immigrated to US and also nurses. What about the safety of medical records on those doctors and nurses' hands? How you will ensure safety of medical records in such a situation. ...
One new nurse was told that her skills are stale and she needs to go through a "refresher" course, which costs $2,000 before she will be able to get a job.
Another one has been looking for work for 6 months with no luck.
I went over there to see if people in other kinds of work eat their young as MTs sometimes do. I thought maybe everyone else was happy and enjoying successful careers. Apparently not.
These are tough times. I think sometimes we generalize so much that we depress each other. N ...
Obama Administration to Announce $975 Million in Health Grants
Feb. 12 (Bloomberg) -- The Obama administration today will announce $975 million in government grants to expand the use of technology in the health-care industry and train medical professionals to use new systems.
The spending, part of the $862 billion economic stimulus legislation passed a year ago, will expand technology use at more than 100,000 hospitals, according to an administration official speaking on the c ...
I just heard tonight from a lady who has been in school studying to become a nurse. She told me basically her schooling is now over. She made a 74.? in pharmacology and you have to have at least 75. She says she has spent so much money, really applied herself and now all that is over. There is only the 1 time, she tells me and she missed it. ...
I am looking into MT programs and wondering about one which advertises that it is geared to nurses: MT@Home Medical Transcription. It costs significantly less than the three programs which I see referenced quite a bit -Andrews, M-Tec and Career Step, but it seems like these programs involve so much material that I already know. Years ago I did transcription for nurses, I've worked in hopitals/home and office environments as a nurse (also years ago) and I worked in pharmaceutical sales ...
Just wanted to dedicate this post to the nurse practitioner who coughed very LOUDLY throughout two 10-minute reports I just did. Just to let you know, there is such a thing called the MUTE BUTTON. Thank you so much for just about blowing my ear drums out. I literally had to pull my ear plugs out so you wouldn't render me deaf. I feel sorry for whoever uses that dirty, germ-filled phone after you hacked a lung into it.
Oh, and BTW, the phrase "carotid bruits" is not pronounced carotid brew- ...
I came across an ad recently catering to unemployed new registered nurses who need jobs badly. I am understanding that the market for RNs and healthcare in general is saturated and then some; however, this article specifically stated "Nurse Medical Transcriptionists." I saw it on All Nurses forum; I don't remember the company . . . BUT a lot of nurses were responding with interest because they can't get jobs. The only thing that was running through my mind was "you have no idea people ...
Just wondering what the majority thinks about these career paths. It seems as though a lot of MTs are moving or considering a move to the RHIT/RHIA world of especially medical coding. I am assuming that the lure of 'work at home' is pulling many in this direction, and I have actually seen where many are questioning whether medical coders are 'paid per code' or 'paid per case' or what? I asked myself what are these people thinking? Has anyone experienced this, ...
and she knew someone who is almost retirement age, having trouble making ends meet, no husband -- now she didn't know if she could spell, but she could probably type -- and figured maybe she could do what I am doing. She didn't get along well with people, so it would have to be an at-home position. ARRRRRGGGGG!!!! and could I help her...
now I have had numerous calls such as these over the years, but from a NURSE!? You'd think at least she ...
I need to find a real job with benefits and insurance so I am thinking about going to school to be a LPN or radiology tech. Anyone done either of these? Did you feel like your MT background helped in the courses? I am 40 so I will have to take all that math and english before I can do anything. Any advice.....? Thanks ...
I have been working in this field for many, many years. When I started, it was a little easier to break into this field. It was in an era when everyone didn't work at home and every company didn't require 2 years experience.
I figured the newbies on this board would know the answer to my question. I have a friend who has been an RN for years and would like to break into the MT field. Being that she has terminology/medical experience, is there any way ...
I have a physician who needs ideas of verbage at the bottom of dictation in which his nurse practitioner dictates notes for him on patient's that they see together. Has anyone provided anything like this before? Any ideas? ...
That was such a clever idea to multitask and eat your lunch while dictating, killing two birds with one stone, as they say. It's so cute and funny hearing muffled words through your chomps and swallows. I wish I could return the gesture and make your day more pleasant too. Merry Christmas. ...
Of course we start talking about everything. Well my local hospital surgeons no longer dictate. Their medical students follow them around and are in charge of keeping track of everything and then later going in and entering it in the computer. This is a HUGE group of hospitals and I am sure impacts a lot of us. Also, my local doctors have begun bringing their computers into the room when your in for whatever, and doing the paperwork right there, on your dime or the insurance companies dime, ...
Not sure if they are saying VSE, PSE or BSE? It's a protocol if a nurse or healthcare worker accidentially sticks themselves with a needle and I tried googling, but I can't find anything definitive.
TIA ...
I am not allowed to think for you!
Sincerely,
The Typing Zombie
I have done three extremely long H&Ps dictated by the same nurse practitioner and OMG. She has dictated the entire physical exam and in the middle of dictating the extremities, she says, "He has been wearing O2 2 liters nasal cannula during the daytime hours." No, go back up and put this in under vitals or respiratory. Just, "There is peripheral edema. Pedal pules are normal. Skin is ...
I'm sick of working my butt off for crap wages. Why can't we strike? Because we work AT HOME. We can't group up and stick up for ourselves. I, for one, has had it. I have 30 years experience. I've watched my salary sink lower each year. If it wasn't for my husband's income, we'd be on the streets right now. I have 2 more hours on my shift but guess what - SCREW IT. I'm done for the night. What am ...
I just got another email from my QA super saying that I should have left a QA note for a report, however I did. This is the third time this has happened to me. I have called support and they say there is nothing wrong, but I only work 3 hours a day (other full time job during the day) so I remember the reports that I type. Is this happening to anyone else? I've am already on probation and this isn't going to look good obviously, but I don't want to sound like I am making ...
When everything after heading is numbers (such as IV FLUIDS 1500 ml, EBL 2000 ml, etc.), do you spell out fifteen hundred milliliters and two thousand milliliters? That is how I interpret the rule of not starting a sentence with numerals, but it really does cloudy the waters and make the op note less easy to read. ...
If you insert a QA marker, how do you close the QA note box at the bottom? Ctrl+C doesn't work. I have to manually click in the report to get it to close. Is there a shortcut? ...
I used to specialize in op notes but for the past 3 years I have not found a company where I could solely specialize in op notes so I have kinda gotten rusty. What I have noticed in the past 3 years is the few op notes that have come my way are by the worst ESL dictators. Is this a trend in op notes now? Does it seem that they are done mostly by ESLs? I was just curious as to other MTs recent experience with op notes, especially those who work is mostly op n ...
especially if you're on a southwestern account and doing editing for the first time. I started with MQ last month and feel very discouraged, mainly for all the minor negative feedback I'm getting. I'd like to know how the other new employees are doing so I can get a sense of where I stand. I keep thinking any day now I'll get a notice I'm fired. Email me directly and dish it out. ...
I had op notes added to my q last week. My line count has went up quite a bit due to the doctors using templates for a lot of op notes and the fact that they say the same things usually. Anyone else end up with more lines doing op notes? ...
I have a chance to get in with an MT company that also offers their ICs a chance at live note taking. It doesn't pay too good, though, less than what the standard reports pay per audio minute. I have never done this. Can someone explain to me how it works? I have heard that "live notes are typed in one pass." What does this mean? ...
Did anyone else get the speech again about pend notes being too long? I can understand why they want us to be "professional" about it but if there is an ODD problem (and there always is!), how am I supposed to tell them? Today, it said referring physician was Alan so-and-so, so, there was no ALAN So and so in the list so, I had to leave a blank BUT, the HPI mentions Albert So and So. Um.... okay, but still no Albert so and so.... Then, back to Alan, so and so, then at the end, copy to Albert ...
