I see nothing wrong with copying if they are at least scanning the copy for errors. My physicians copy all the time, especially for FH, SH, PMH, diagnoses list, etc.

This paragraph is where the bigger problem lies, IMO: Experts suggested that copying signifies a shift in how doctors use notes - away from being a means of communication among fellow healthcare providers and toward being a barrage of data to document billing.


This is a bigger issue because the note, which is required for billing, has become more about billing, than about the actual patient.

Yes, they copy. We copy and get paid.

I have great accounts as well as physicians contacting me to handle their transcription. One account is so very lucrative (handled his partner's transcription for years), that all you do is pull up prior report, change date, and transcribe 1-2 sentences at most and get paid for 4-5 pages of transcription.... Problem is that I had to turn it down and others like it because I am too buried in other work and cannot find a transcriptionist who I can rely on to perform quality work (always get mistakes such as he versus she, right versus left, AND CORRECT DRUG DOSES (which is why the last service was fired, as I was catching mistakes of drug dosages high enough to kill 15 patients), etc., etc., etc. This one account is over $200,000 per year......

The family practice I go to is using this. The doctor asks questions and constantly inputs into his PC tablet as we speak. Its not looking good for the MT field. Anyone else worried? I have been corrected by QA twice on this one. The first time it was changed to capitals (Electronic Medical Record), so I started typing it that way. However, recently I had it changed to lower case (electronic medical record). Both times the sentence was the same... "Detailed in the electronic medical record."   I had always typed this lower case until it was changed the first time. I never did understand why it should be capitalized. For experience, all they ask is basic computer experience, knowledge of medical terminology, and a h.s. diploma.  Hmmmmm...I've had the terminology certificate for a year, plus a completion certificate for MT and several specialty certs, and I passed the RMT exam. Think I ought to take a shot at this even though I have no prior experience with filing, records, etc.? ...

I would advise EVERYONE to get a copy of their medical records and view them.  I had to get copies from my  PCP (cost me $15.00, but worth it), and I found soooo many mistakes.
