So daughter had to have sports physical last night.  We saw the NP rather than MD.  This woman was *way* over the top.  Checking Tanner stage on a 15-year-old who very obviously has hit stage V???  For a sports physical????  Ugh.  Dd was *ticked* and I was so shocked I just froze.   

Anyhoo - NP informs me dd needs Tdap.  Uh... no, she doesn't.  She had freshman physical last year and that was when the Tdap needed to be updated, so I'm sure it was.  "Well, it's not in here." - pointing to handy dandy laptop.  Well, I say, how 'bout you check the hard copy of the physical form that this office made? 

NP:  Oh, apparently that wasn't scanned in either. 

Me:  Well, I guess you'd better check hard copy file.     

NP:  Oh, well I'll have to have the girl pull that from the basement but that's going to take a while.  Why don't you just get a copy of physical form from school?

Meanwhile, she's pulled out the syringe to give her another shot.  Not only no, but heck no.

So now it's apparently my responsibility to maintain and update *their* medical records? 

Heaven help folks who still think medical professionals are infallible and don't dare to question things.  I'm not seeing this as a cost-saving measure when tests/shots are duplicated because they forgot to click a box. 

And I guess I'm going to have to get back in the habit of keeping a hard copy of every darn thing.

that the EMR is fine, but the office staff who are supposed to do that work are not doing their job. I think that office would be hearing from me.

This is a wake-up call for all of us. We had better keep tabs on our own medical records, as it seems that errors are going to increase, and the scariest part of it, IMHO, is that those office care providers are not going to think outside the box, but are going to depend on whatever the laptop has to say about it...

ER visit a year or so ago. Took copy to my PCP later and he said, did you notice what is on this. Had under diagnoses HIV as one of them. Totally wrong, never had. I might have let that slipped but this to me was more than a usual mistake. Talked to ER manager and told him he should perhaps find out how this error came into play.

I'll bet this is more common than we know, and I expect it will get worse. I pity those who have no idea what certain medical terms mean and wouldn't know how to interpret their reports (or even get access to them) in order to keep their records accurate. They should be able to trust their medical team to take care of this, but they really can't.

I've been copied on EVERY SINGLE THING in my medical records. Tried to tell my family they should too but they didn't listen, then my niece found out they have her in her records as an alcoholic nonsmoker with severe COPD and PAH, when she is a smoking nondrinker with no apparent lung problems or PAH.

that's terrible! Was the oversight due to EMR? - Did you sue?

and make the biggest stink about the whole thing, and I'd DEMAND to see the billing charges to make sure you weren't charged for that shot, and charge them or demand fee reduction for any copies you have to fetch.

The Tanner stage bit had nothing to do with the record. The other problems would have occurred with a paper record.

My daughter had a similar experience during her sports physical. She reported that they asked her about her use of alcohol, illegal drugs, sexual activity, birth control, AIDS exposure, gun use in the home, etc. She answered no to everything, and the NP reminded her that their conversation was confidential and she could speak freely. My daughter reiterated that she didn't do any of those things, and apparently the NP replied with a skeptical "okay". The obligatory sports physical turned into something of an inquisition. I guess I'll go in with her next time. :)

Dictated:  On examination, there is extensive scarring noted down the left leg secondary to previous skin grafts.  There are multiple areas of crips and pits secondary to the scarring.  No infection.  No active ulcerations. The original traumatic injury ("mangled extremity necessitating significant reconstructive surgery") was 10 years ago.  This pertains to residual appearance, not related to any recent postop, dressings, etc.   Anyone?  Thank you ...


For Anyone That Works On EMRs Or Epic, Do You Still Continue

to work in your regular platform such as DQS or do you have to download an entirely new system to work in a hospital EMR.  My hospital is on DQS and going to EMR or Epic so I am not sure if that means all my short cuts etc are gone or we continue somehow to use both systems.  I doubt you can use short cuts in EMR but again I have no idea and I dont think you can move your short cuts out of DQS to another system and again I dont know.  Seems like I dont know much this morning at al ...
