I am hearing a new one on emergency room reports. They are saying what sounds like "An INT was placed." What is the INT? It is placed with cardiac monitors and such. ...
I know a comma is to be placed after the year, i.e., On March 27, 2011, she underwent an operation. But, what about when formatted this way? 3/27/2011? Do you still place a comma after the year?
TIA!

Thank you! ...
If they offer you a "placement change" (which sounds a little fishy since all of the regions have no work as far as I can tell)...do you have to accept the change if it means a schedule change (i.e. shift)???? Why should we have to change our shifts????? Geez.... ...
I get these types of sentences constantly and always question my comma placement.
"We will see her back in the office following her injection and if she does not improve, we would consider surgical intervention at that time."
My question is whether there should be a comma either before or after "and." I know a comma needs to separate 2 independent clauses but the introductory phrase in the second clause throws me off.
Any suggestions would be greatly appreciated :) ...

Anestasol Steri-Strips were applied.

Does anyone know anything about this company? I sent my resume and got a response right away. It seems like they might be a 'head hunting' agency for transcription of different sorts. They want me to pay a one time fee of 75.00 to join and then have contact with all of their clients for jobs. I'm a little leery about them, so what does eveyone think? Any thoughts are appreciated - positive or negative..... Thanks! ...
For the eye stent part of the dictation, it sounds like the doc is dictating "The (s/l oneone or odeon lens) was used to view the angle structures". Thanks for your help. ...
I am a new MT who has only been in the business since March. I still review every single one of my reports...I did stop listening to them again since I am comfortable with that part...My question is what is a normal amount of time to auctally stop going over the reports again?...I do understand sometimes that it is necessary to sometimes go back over them. Problem is work is picking up and I simply can not keep spending too much time going back over them, not to mention that it dips into p ...
"Bilobed microsystem is noted by the pancreas with abduction of the body and tail immediately adjacent to normal pancreatic duct. "
The sentences immediately preceeding this are "Common bile duct and intrahepatic ducts appear normal. No obstructions are noted." The sentence after it is "Early developing cystic neoplasm is considered likely; typically these are benign or nonaggressive."
So it would go: "Common bile duct and intrahepatic ducts appear normal. No ob ...

Sometimes, we can get into bad habits, and I was wondering about your thoughts on the sentence below regarding comma usage (I tend to overuse commas)
"I advised a low-fat, low-cholesterol diet, and, for your information, your body mass index today is 40.2, which, by definition, places you into the obese range."

Dictator says this:
REVIEW OF SYSTEMS: There was no concerns about eyesight, cardiovascular, GI, GU, musculoskeletal, dermatologic, endocrine, hematologic, (s/l renalcologic) systems.
I cannot find anything to verify if he is actually saying renalcologic or maybe craniologic. It's about a 2-year-old girl with a possible genetic disorder.
