This disclaimer was at the bottom of a note:
Dragon medical voice recognition software was utilized in completing this note. As a result, errors may occur. When identified these errors have been corrected. While every attempt was made to correct errors during dictation, errors may still exist.
But if an actual human leaves an error we get dinged, demoted, decreased in pay, not to mention the wrath of QA. Perhaps there should be an error limit for Dragon. If we have too many errors, we get fired. How about doing the same with Dragon? Bring back the humans who actually know what the doctor is saying, knows the difference between sound-alike words, and knows how to punctuate a sentence. Sheesh!
