finishing reports with "end [patient name]" after dictating any CCs. IIRC it had to do with a southern accent that might not translate well.
He kept doing it.
One day he dictated a report with several CCs, then finished with "end [Mrs. Schnicklefritz].
The MT heard that as "AND Mrs. Schnicklefritz," and the patient got a copy. Problem was that Mrs. S. had died.
He stopped after that.
