Question regarding patient v the patient - in transcription

This is driving me crazy.  I keep getting marked down for putting the patient when the dictator says the patient and it is marked out as being patient (without the) instead.  This is happening numerous times.  Can anyone tell me where there is documentation that "patient" is to be transcribed instead of "the patient."  I asked my supervisor about this constant dinging by QA and they did not cite anywhere this is correct.  I am so tired of seeing this on all my work and it is not what the dictators are saying.  I just need someone to point out where in the BOS or in our industry where this is definitively that we need to put "patient" and not "the patient" even if dictated "the patient." Thanks for your help.
