We used to never be counted off for added/missed "the" or "a/an" - insane

It's almost always subjective anyway. Some people can hear a swallowed article better than others, plus depending on how fast the dictation is played, etc.

I do have to sometimes add "the" to "the patient" just at the beginning of a sentence even when it's not dictated on some accounts, but that's it as far as "the patient" vs. "patient."
