You said they asked you "to put down your start time, stop time, day, # of hours."
Using your example:
Start time: 12
Stop time: 8
# of hours: 5
Day: 7/2/17
If they wanted you to punch a clock, I'm thinking they wouldn't need to ask for the "# of hours" or the day. They'd already know that based on your punches.
