Doctors who keep words to a minimum - anon

I have a doctor on one of my accounts who LOVES to dictate as few words as possible.  VR picks it up really well; that's not the problem. 

How do you handle thpse reports that need to be fleshed out so much?  Personally, I don't think he should be on VR since you need to add so many words that the pay rate becomes next to nothing for his reports. 

Example: 71 year old. Followup. Pain. Arm.

Flesh it out or leave it be?

It's not our job to decide what he REALLY wants to say. Besides, maybe next time he might realize just how ridiculous his dictation really is and be a little more verbose when dictating.

That's appropriate style AS LONG AS it's consistent - throughout at least report sections.

Of course, we all know it so seldom is. If it's sloppy in favor of full sentences, as most are, I fill in and yank out "extra" words within sections, such as PE, for conformity. If it's a mixed style that specifically favors phrases, though, I leave it the way it was dictated. Either way, as said, ultimately I'm trying for a professional product in the dictator's chosen style.

My problem is the opposite - my doc loves to hear himself talk! He dictates every normal organ system, as well as the abnormal on every report.

I was told a very long time ago not to change dictator style. This is obviously his style, and clipped phrases/clipped sentences are permitted in medical transcription. Transcribe as dictated unless some tweaking is needed for clarity.

I was always told that too. At the same time, we're told to edit grammar to make it more readable. I wasn't sure which way I should go with it. It definitely makes more sense to keep it as is for VR.

I am doing a dictation report in Urology and in the list of impressions, the doctors dictates: Hyponatremia, probably secondary to hyperglycemia. (sounds like he says "transficate" artificial and "transficies" amd Zarololyn. Is it acceptable to flip the word order for sutures to avoid starting a sentence with a number? If "quarter percent Marcaine" is dictated at the beginning of a sentence, I would transcribe "Marcaine 0.25%." Can I extend this rule when "zero Dexon" starts the sentence to transcribe "Dexon 0"? ...
