Acute care - define please! and Standard 4 - McD MT

I have been an MT for 16 years and I am still not totally sure what "acute care" means and it is on almost every ad.  Does this always include Ops notes?  I also can only make educated guesses about what the Standard Four are too.  I am frantically looking for a job - no work at my current one - and I need to understand these terms better so I don't waste my time.  I have no Ops experience and seems everyone wants this.  Discouraging.  I have done only ER work and a bit of clinic work and can't find anyone who will hire me.  If any ideas on who is hiring, please include that info!

History and physicals, consultations, operative notes and discharge summaries. Sometimes the emergency room reports will be included. The first 4 are acute as well as the standard 4.

If you are seen in a clinic that is usually followup exams and ongoing problems. Acute care is something like "now" and therefore the hospital setting, car accidents, heart attacks, strokes, and the like. So you do the basic 4, HP, consult, DS and OP notes. ER is only the introduction to acute care. Hope that helps to explain. I much prefer OP notes myself and have worked in hospitals and clinics. Clinic work is too slow-paced and boring for me, no challenge. You also have to be familiar with all specialties like OB/GYN, Ortho, Ophth, Neurosurgery, and the like. The broader your experience the easier to find a job.

at home with your 16 years experience and Google, you'll start out slow but should be able to produce clean reports pretty much right out the door. Sign on with a company that allows you to search patient and dictator reports for terms.
You are kidding, right- asking this person to - misrepresent their abilities?

Why would you ask a person to outright lie and say they have acute care experience if they don't? Acute care is very hard to learn. That would be like telling someone who can type just to start transcribing. Surely you jest!

How else does one get into "acute care" if not in this way. At least she can try. Think positive!
We all started out with no experience in any field.

You do not pad your experiences in this work, never. If you do that, might as well lie about everything else. What others suggest is underhanded and I would hope anyone who outright tells a lie gets caught in the same.

But with 16 years, I'm betting on her. BTW, this is the way I got my first couple of home jobs after training in-house, and all reports went in clean and polished. As I learned, speed improved tremendously, quality only slight room for improvement.

As a relative newbie MT, I'm not certain what "acute care" means. Does it mean basically all reports that are from doctors in the hospital setting? That is, would any hospital operative note, hospital progress note, and all discharge summaries be considered "acute care"? I know a clinic note is basically a note by an outpatient doctor, right? And it's basically about more minor medical problems albeit could be chronic rather than "acute." ...

I've been in transcription since 1998, doing clinic work for about 13 of those, then moving to acute care consisting of H&P, consults, and discharge summaries....but not op notes. I see so many jobs for acute care work, but they only want people who've done all four. My question is, how do you go about learning to do op notes in order to obtain one of those jobs? I've been tempted to fib and say I have experience just to get the job and then figured maybe I could learn on the job ...
