Patient has hip effusion, fever, inability to bear weight, and elevated CRP and sedimentation rate. Doc is saying, although septic arthritis is in the differential, because of some tenderness down the thigh, they are also concerned about an extra-articular source of infection, including proximal femur s/l **osteo-**. I'm sure I need to expand this but not sure what to put since many things begin with osteo-, such as osteoporosis, osteonecrosis, osteoarthritis, etc. Doc also says as part of plan that they'll get a "fast **osteo-** sequence MRI" of the patient's pelvis and femur. Anyone have an idea of what's the most likely expansion for this? Thanks in advance.

The MRI would be fast spin echo (FSE) MRI.

I think hip effusion is related to osteoarthrits but can't guarantee this is what your doc said.

Thanks, but definitely saying "osteo-" again. - jld

I know this doc is wording it weird. I knew that she was talking about a fast-spin echo MRI but is definitely saying "a fast **osteo-** sequence MRI," and since this account is pretty much verbatim except for correcting grammar and such, I was just going to expand the "osteo-" part and leave the rest as dictated. I was thinking of osteoarthritis, too, except that since she's saying they're looking for a possible extra-articular cause BESIDES septic arthritis due to the extension of symptoms down the thigh, then I figured this was to check for something that could affect the bone other than a form of arthritis, but I'm just really at a loss at this point. I think I'm just going to blank both spots. Thanks for your help, though.

consider osteomyelitis - nm

Ahh, I bet that's it! Thanks. I already blanked it, though.--nm - jld

did you try "osteopenia" ?

I put "osteo" when they say that... - ...to many other possibilities, but...

Most of the time I know what they mean by the content, but I don't expand it anyway. Could be osteomyelitis, -penia, -arthritis. I decide to leave it to the reader, and the dictator to be clear. If they're considering antibiotics, I assume osteomyelitis, but I still don't type it. Call me defiant, let them expand it.
