should these be capped? Glidewire and Glide catheter, glidewire and glide catheter, or glidewire and Glide catheter?

I would say no in this case if it is dictated as "a" glide wire, etc. - But I have seen it capped as name brand. SM

I guess decide according to the context of the note and see what your QA says, and also ask them if they're truly helpful and not punishment oriented.
