Hard to understand doc is performing a mastectomy with what sounds like a "ski margin pen".  Any idea of what kind of marker pen he is using?

Not just saying skin-marking pen? sm - Txczech

Used a lot in plastics etc.

Probably a "surgical marking pen" (used to make the marks where the incisions will be...)

I would go with skin-marking pen - SSMinnow

He is marking the skin, not marking the surgery.

Question About QA Marking ISR Expansion

Just wondering if anyone else has had this happen. I got 0.25 taken off for the dictator having dictated "temp" and the report having "temperature." Had I typed it from scratch I would have typed temp, but the ISR did it and it said "temperature," so I did not change it. Are we actually supposed to shorten the expansions perpetrated by the ISR? Know villiform is a real word, just have not heard it in this context:  "There was a small false passage at the 6 o'clock position in the distal prostatic urethra; I suspect this is the area that presented difficulty with insertion of a Foley catheter and villiforms and dilators." I'm having trouble deciphering a few words in this sentence dealing with ankle surgery to remove intracuticular sutures.  "Tourniquet was placed (about/above?) the left thigh and a (s/l bump) placed beneath the left hip.  The lower extremity was then prepped and draped in sterile fashion."  I can't tell if he's saying about or above, although I'm not sure if above would make any sense in that sentence, and I'm not sure if bump would make sense, either. ...

This is probably a ridiculously easy one, but I don't have any QA to tell me anything, so I don't know if I've been doing this right or not (though it feels like it's wrong). The questionable term I come across for this one account is "including risks (s/l inherent) surgery." I feel like it's missing a word there, like risks inherent IN surgery or something similar, but doctor repeatedly only says "risks inherent surgery" in that order. Is she actually saying something ...
