A paraneoplastic phenomenon is a disease or symptom that is the consequence of the presence of cancer in the body, but is not due to the local presence of cancer cells. A large number of these phenomena are mediated by the immune system.
The direct clinical manifestations of cancer are usually due to local effects of tumor growth, either in the primary site or at a distant site, or are nonspecific such as anorexia, malaise, weight loss, night sweats, and fever. The term paraneoplasia, which means “alongside cancer,” has been commonly used to denote remote effects that cannot be attributed either to direct invasion or to distant metastases.
The most common cancer associated with paraneoplastic syndromes is small cell cancer of the lung, probably because of its neuroectodermal origin. Other neoplasms commonly associated with paraneoplastic syndrome include carcinomas of the breast, ovary, other adenocarcinomas, lymphoproliferative diseases (especially Hodgkin's disease), and thymoma.
Paraneoplastic anemia can also be caused by hemolytic anemia related to warm- or cold-reacting antibodies in the setting of B-cell malignancies, especially chronic lymphocytic leukemia and lymphoma. Treatment is directed toward the underlying lymphoproliferative disease.
